SUPPORTS PROGRAM

The Supports Program is an initiative by the Division, included in the Comprehensive Medicaid Waiver (CMW) approved by the Centers for Medicare & Medicaid Services (CMS) on October 1, 2012. The CMW aims to reform Medicaid services statewide, emphasizing community-based support and enabling New Jersey to access increased federal funding.

The Supports Program offers essential supports and services to adults aged 21 and older who live with their families or in unlicensed settings. It is designed to enhance services for adults with developmental disabilities and significantly decrease the waiting list for supports and services. All enrolled participants will receive employment and day services, as well as individual and family support services tailored to their assessed needs. Individuals and their families can choose the support options that best fit their requirements, with the guidance of Support Coordinators who assist in developing an Individualized Service Plan and connecting them to suitable services.

Except for those enrolled in another Home & Community Based Setting (HCBS) or Managed Long Term Services & Supports (MLTSS) program (including the CCP), all adults eligible for both Division services and Medicaid can access the Supports Program.

SUPPORTS PROGRAM + PRIVATE DUTY NURSING (PDN)

In situations where an individual has been evaluated by the Managed Care Organization (MCO) and determined to need Private Duty Nursing (PDN), but would benefit more from the services available through the Supports Program rather than Managed Long Term Services and Supports (MLTSS), they can be enrolled in the Supports Program and receive PDN through Medicaid. However, they would not be enrolled in MLTSS, as federal regulations prevent participation in more than one waiver program simultaneously.

To qualify for the Supports Program along with PDN, an individual must meet the criteria outlined in Section 5.1 and be eligible for PDN services. The individual's MCO will perform the NJ Choice Assessment to evaluate their level of care for PDN. If the individual meets the necessary level of care, the MCO will then conduct a further assessment to determine the extent of PDN services they can receive. The enrollment process for the Supports Program and PDN is detailed in Section 5.2.1. Once eligibility for both the Supports Program and PDN is confirmed, the MCO and Division will collaborate to coordinate the necessary services.

COMMUNITY CARE PROGRAM

The Community Care Program (CCP) is an 1115 Medicaid Home and Community-Based Services (HCBS) waiver program that enables New Jersey to receive federal matching funds for a range of approved services and supports. This program helps Medicaid beneficiaries live in the community and avoid institutionalization.

The CCP is essential for the Division's efforts to provide community-based services to individuals with intellectual and developmental disabilities. Without it, New Jersey could only utilize Medicaid funding for services if individuals were living in institutional settings. The federal government allows states to establish waivers like the CCP to assist individuals with specific service needs in avoiding institutionalization and either returning to or remaining in the community.

To enroll in the CCP, individuals must be eligible for Division services, meet the clinical level of care criteria for Intermediate Care Facility for Individuals with Intellectual Disabilities (ICF/IID), and comply with specific Medicaid income and resource requirements.

The CCP offers vital supports and services for adults aged 21 and older living in various settings, whether with family, in licensed residential facilities, or in unlicensed arrangements. All enrolled participants can choose from approved waiver services and providers, allowing individuals and their families the flexibility to select options that best suit their needs, with support from Coordinators who help them develop an Individualized Service Plan and connect them to appropriate services.

Individuals enrolled in the CCP are not permitted to participate in any other Home & Community-Based Services (HCBS) or Managed Long Term Services & Supports (MLTSS) programs, including the Supports Program.

Community Care Program Waiting List Summary

Individuals eligible for Division of Developmental Disabilities (DDD) services can enroll in the Supports Program at any time. The Community Care Program (CCP) waiting list allows individuals and their families to express a potential need for CCP enrollment, but being added to the list does not guarantee enrollment. Eligibility and need will be assessed when an individual reaches the top of the list.

Eligibility Criteria for the CCP Waiting List

To enroll in the CCP, individuals must meet DDD's eligibility requirements and be determined to require an Intermediate Care Facilities for Individuals with Intellectual Disabilities (ICF/IID) level of care. This means that, without CCP enrollment, they would qualify for services in a state-run institution for those with intellectual and developmental disabilities.

Details about the waiting list and enrollment procedures are outlined in Division Circular 8 (N.J.A.C. 10:46C). Generally, the Division processes the list in chronological order.

Individuals can request to join the CCP waiting list if they:

  • Are 18 years or older,

  • Are Medicaid eligible, and

  • Have been determined eligible for services according to Division Circular #3 (N.J.A.C. 10:46).

Alternatively, they may also qualify if they:

  • Were born before January 1, 1997,

  • Were determined eligible for services before January 22, 2013, and

  • Are Medicaid eligible.
